Vlan add culprit guilty as charged.

I think the biggest blunders I've ever done is either getting a dd input
output confused on a prod system. Thank goodness for backups or locking
everyone out of a remote console server with a mis-configured TACACS
configuration.

Always remember to remove the exec-timeout when doing tacacs...

To this day I still read the man pages to ln to ensure I don't wipe out the
source file.
