The ISP Billing is the easy bit.  Most (all?) ISPs have the ability to "unmeter" traffic to/from specific IP addresses.  If you know what the IP address(es) of the carrier end is then unmetering is very straightforward.   

I also can't see the worry about the data anyway - my iPhone prefers WIFI in my house to 3G data. 

Femtocells need a bit of a disclaimer though in terms of reliability.   If you're relying on it for coverage then you'd need to consider what happens in the case of power failure etc.  How would this change your ability to communicate if your ADSL modem was down and that took out your mobile coverage.    Most people _assume_ mobile coverage = battery backed and "carrier" grade.
