Hi Paul,
          Having spent some time in the past hunting for RJ21 to <various> cable assemblies in the past, I suspect you're going to struggle, your best bet is probably to get an RJ21 pigtail and a 24 port RJ45 patch panel and punch it down yourself.

I have tried a number of suppliers but availability of something like this seems to be minimal, some have said they can get it in 1-2 months, others say no, I can find them online from the UK but would rather purchase from an Australian provider if I can.
 
Any suggestions on who would have such a cable would be appreciated.
