If the sender or the attachment is really important to me, I save the
attachment to disk and unwrap it using the tnef utility.

Otherwise I either junk it or reply with a request to deliver the
attachment using a standard mechanism or make it available to me for
download somewhere.

tnef can be integrated into my mail client with a little work, or could
be integrated into my file manager, but I get so few attachments in this
format (that matter) that I haven't yet taken the time.
