[AusNOG] Anonymous Threatens to Expose Data from an Australian ISP

Looks to be at least two failures here.

AAPT cancelled a hosting/colo/dedicated server/other plan with MelbourneIT, and didn't remove the data before handing the keys back.

MelbourneIT closed the service, and didn't delete the data, or left the server online and unattended.

It's all speculation until proven otherwise, but there's a lesson to be learnt in there somewhere.
