+ laptop with a DB-9 connection, or otherwise testing to ensure that your USB to console device functions correctly under the specific OS version you're running! Nothing worse than a contractor/tech turning around and saying he has console access, only to find his drivers don't function under a 64 bit OS in an emergency scenario! It's a very different thinking you're prepared and actually testing to ensure you're prepared!   :)

To be clear, if you don't have a jump bag with all of this stuff in it that you carry in your vehicle/on your person at all times you're a failed change/disaster waiting to happen.

Plan for the worst and don't be the guy who has to back out a change because he forgot a cable.  Implement as best you can, replace when possible (everything is redundant and you can get downtime on that mission critical cable, right?).

Crimping tool, RJ45 connectors, length of cat5 cable. Handy items to have in the toolkit or floating around in the back of the car. ;-)
