It might work, but you definitely need the Mode Conditioning Cable (to make
sure the single mode optics in the transceiver launch down the side of the
multi mode core). But you are right on the limit for a single fibre run.
Each patch will add loss - and with six it all adds up. Unfortunately most
enterprise network equipment is pretty much go or no-go as far as
determining whether it works. You probably either need a OTDR test done on
the run, or run things and 100BaseFX.
